The Prevalence and Factors Associated with Respectful Maternity Care in Musanze District, Rwanda Healthcare Settings.

Abstract
Background:
Respectful Maternity Care (RMC) is an essential strategy to improve mothers' positive experiences during childbirth. However, there are few studies about the prevalence of this care in Rwanda, especially in Musanze District.
Objective:
The main objective of this study was to determine the prevalence of Respectful Maternity Care and identify its associated factors in Musanze District, Rwanda, healthcare settings up to May 2024. Methods: This is a health facility-based cross-sectional study conducted among women delivered in Musanze healthcare facilities up to May 2024. Random sampling was used to select health centers and included all participants who satisfied the inclusion criteria until the predetermined sample size was reached. Multivariate logistic regression was used to identify factors associated with the provision of RMC. The results were reported as adjusted odds ratios with the 95% CI. Variables were proved statistically significant based on p<0.05.
Results:
In this study, 335 participants were enrolled. The prevalence of Respectful Maternity Care was found to be 65.1% (95%CI :59.7 - 70.2). Being employed compared to unemployed [AOR=17.75, 95%CI :8.06 - 39.06, p<0.001], primiparity compared to multiparity [AOR=5.15, 95%CI :2.07-12.79, p<0.001], cesarean delivery compared to those who delivered vaginally [AOR=6.00, 95%CI:2.40-15.03, p-value=0.003], delivery at health center compared to the delivery at the hospital [AOR=3.72, 95%CI:1.41-9.83, p-value=0.008], daytime delivery compared to delivery at night [AOR=3.11, 95%CI:1.52-6.37, p-value=0.002], and using other insurances (Military Medical Insurance, Rwanda Social Security Board, Radiant…) [AOR=4.46, 95%CI: 1.88-10.61, p<0.001] compared to Community Based Health Insurance, were found to be statistically significant factors associated with higher Respectful Maternity Care during childbirth.
Conclusion:
The prevalence of Respectful Maternity Care in Musanze district is low. Being employed, health insurance type, primiparity, cesarean delivery, delivery at health center and daytime delivery are factors associated with Respectful Maternity Care. These findings call upon policy makers to involve different stakeholders to come up with interventions to uphold the rights women’s right during childbirth.
